- Develop and implement individualized Support plans.
- Gather and maintain information to complete special reports to include coordination of care and quarterly reviews.
- Provide transportation for program members to and from community activities.
- Serve as clinical resources in electronic health records.
- provide services in compliance with Department, City, State and Federal regulations and standards.
